New Patient Registration

non_nhs_services1The doctors welcome new patients who live within our practice area. A map of the area is available at reception. All patients (5yrs and over) are offered a new patient registration health check.

You may choose to see any of our doctors and we will try to offer appointments with the doctor of your choice, timetable and holidays permitting. All doctors have access to your computerised medical record. 

As it may time some time for your medical records to be forwarded from your former practice, all new patients will be asked to complete a registration questionnaire (below) and attend an appointment with the practice nurse. 

Registration days at the practice are:

Monday to Friday from 1:30pm-6:15pm

You will need:

  • Your NHS number
  • One recent proof of address (eg a utility bill dated within the past 3 months)
  • One proof of residency (eg passport, Home Office papers, a full UK drivers' licence)

You may print off the registration form and registration questionnaire. Please fill them out and bring with you when you come to register.
